We are making some changes to the way we invite our patients’ for chronic disease annual reviews.
From now, we aim to review all relevant patient’s in the month of their birthday, calling them in 1 month in advance i.e. during May we will remind patient’s born in June to make an appointment to see the Nurse in June. We will try to do all reviews at the same appointment to avoid patient’s having to come back. This should make it more convenient and easier for our patient’s to remember when they need to call the surgery for an appointment.
For further information please speak to reception.

NEW YEAR NEW YOU– please remember we offer a Smoking Cessation clinic at the practice. Please ask for further details at reception.
DNA Appointments (Did Not Attend)
Our practice continues to experience a high volume of wasted appointments due to non-attenders; this impacts the availability of our appointments which is frustrating to many of our patients.
We therefore ask that you contact the Practice as soon as you are aware that you are either unable to attend your appointment or your appointment is no longer needed.
Text Message Reminders
We now have a text message reminder service set up. This allows patient to receive appointment reminders via text messaging and you can also cancel your appointment free of charge by texting ‘CANCEL’. Please update your current mobile telephone number to enable to access this service.
Working to Become Dementia Friendly
Hebburn as a community is working to become Dementia Friendly and our Practice is proud to say we are too!
People living dementia want to remain independent for as long as possible and they want to have choice and control over their lives through all stages of their dementia. The aim of Dementia Friendly Communities is to improve inclusion and quality of life for people with dementia by creating a town which is accountable to people with dementia and their carers.
